Registration forms

 

To provide a better service, we track validation errors that might occur upon user-registration. We do not track any personal information, just the names of the fields which were filled out incorrectly.

Our website´s registration form requires users to give us contact information (like their name and e-mail address) and demographic information (like their sex and age). We use customer contact information from the registration form to send orders and samples when requested. This website collects information only for internal purposes. Personal information will not be shared with or sold to any third party (outside the Coloplast Group).

Ordering products and samples


You can order either complimentary Coloplast samples, or larger quantities of items which will be chargeable. To order either, you will need to register as a Patient or a Healthcare Professional first. This process should only take a short while, and you can then login and place a prescription order or sample request. Healthcare Professionals may wish to have samples delivered to their workplace.

If you are requesting products which you need to pay for, you will need to use a valid credit card on the relevant pages, or our Charter team will call you back to take payment. An on-line secure payment environment is included in this web site.

If you are requesting one or more product samples, please note that there is standard number of each product which is allowed in our sample packs.

Agreeing to these terms and conditions confirm that you are happy for Coloplast Limited to hold personal information about you on their database in order to supply you with products, information and services and keep you up to date with the latest products and improvements.
